چکیده

The cyclic process of proliferation, secretory differentiation, (pre)decidualization, menstrual degeneration and regeneration in premenopausal endometrium is controlled by oestrogens and progestogens, and is mediated through binding to their specific receptors. Oestrogen (ER) and progestogen receptor (PR) presence and concentration indicate not only hormonal influence on the specific cells but also the capacity to respond to these hormones. ER and PR concentrations in endometrium vary with the phases of the menstrual cycle, but their relationship to cyclic changes in histomorphology and hormonally modulated tissue factors in the specific cell types has not been extensively studied. There is some evidence that these changes reflect functional processes resulting, at least in part, from interactions between the different endometrial cell types [1].

Relations between bleeding pattern, endometrial histology, and oestrogen treatment in menopausal women.

Vacuum curettage was performed on 348 women who had received various regimens of oestrogen treatment for an average of 9.7 months for climacteric symptoms. In 62 cases (18%) the specimens were unsatisfactory for histological assessment; among the remainder, however, they showed a normal endometrium in 257 cases (90%), cystic hyperplasia in 21 (7%), adenomatous hyperplasia in 7 (2%), and endomet...

Oestrogen receptor in glands and stroma of normal and neoplastic human endometrium: a combined biochemical, immunohistochemical, and morphometric study.

Cryostat sections of unfixed human endometrial tissue from normal, hyperplastic, and carcinomatous endometrium were assayed for oestrogen receptor content using a radiolabelled oestradiol binding assay with isoelectric focusing to separate receptor proteins. The proportions of myometrium, stroma, and glandular tissue were estimated in adjacent sections by point counting. Further cryostat sectio...

Hormone replacement therapy and the endometrium.

Modern hormone replacement therapy (HRT) regimens contain oestrogen and progestogen, given either in a cyclical or continuous combined manner. Most endometrial biopsies from women on sequential HRT show weak secretory features. Approximately 15% show proliferative activity, although this figure may be less if more than nine days of progestogen is given in each cycle. A small proportion will sho...

Progestogen intolerance and compliance with hormone replacement therapy in menopausal women.

It is vital that we maximize compliance if patients are to receive the full benefits from hormone replacement therapy (HRT). One of the main factors for reduced compliance is that of progestogen intolerance. Progestogens have a variety of effects apart from the one for which their use was intended, that of secretory transformation of the endometrium. Endometrial effects vary between individuals...

Oral Contraceptives Containing Only

Oral contraceptives containing only a progestogen have had a chequered career. In 1965 trials in Latin America showed that very small amounts of a progestogen, chlormadinone acetate, when given without any added oestrogen had a strong antifertility effect. Not all progestogens are suitable for use alone. Indeed with norethynodrel, the progestogen most commonly used in the late 1960s, the amount...
